What to Bring
Photo ID
For the patient (or parent/guardian for minors).
Insurance card
If you have orthodontic coverage, bring the card so we can look up your benefits.
Referral or X-rays, if you have them
Not required, but recent dental X-rays or a referral note can save a step if your dentist already has them.
